Go to website: https://visawebapp.boca.gov.tw . Fill out the application form online and print it out. Make sure the application form shows a bar code on it. The application form must be signed by the applicant, either one of the applicant’s parents, or legal guardian. |

Paste on the application form two color passport-size photos with a white background taken within 6 months. |
| Passport and one photocopy of the passport |
The passport must be valid for at least 6 months and there are blank pages left in it. One photocopy of the passport bio-page including the holder’s picture is required. |
| Original and one photocopy of health certificate |
- The health certificate should be issued within 3 months by one of the local hospitals designated by the Centers for Disease Control of Ministry of Health and Welfare of the R.O.C. (Taiwan) or a foreign hospital. Health certificates issued by foreign hospitals must be authenticated by an R.O.C. (Taiwan) overseas mission.
- Visit the website of the Centers for Disease Control at https://www.cdc.gov.tw/En for health checkup items and the list of designated local hospitals.
- Applicants of the following categories are not required to present the health certificate:
- minors under age 6;
- both the visa applicant and the family member(s) currently investing or employed as white collar worker(s) in the R.O.C. (Taiwan), whom the applicant intends to join with, are from countries whose citizens are eligible for visa-waiver entry into the R.O.C. (Taiwan).

| Original and one photocopy of Certificate of Residence in R.O.C. Alien Permanent Residence Certificate, Alien Residence Certificate, or various kinds of residence permits issued by the National Immigration Agency |
- One copy of the Household Registration Certificate issued within 3 months is required if the individual, whom the applicant intends to join, is an R.O.C. (Taiwan) citizen with household registration.
- Original and one photocopy of Certificate of Residence in the R.O.C. (Taiwan), Alien Permanent Residence Certificate, Alien Residence Certificate, or other kinds of residence permits issued by the National Immigration Agency are required if the individual, whom the applicant intends to join with, is an R.O.C. (Taiwan) citizen without household registration, a foreigner having obtained APRC or ARC, a Hong Kong or Macau resident, or a Mainland Chinese citizen. The documents must be valid for more than 6 months.

| Original and one photocopy of proof of parents’ marriage |
- For applicants who are under age 18 and are the children or grandchildren of an R.O.C. (Taiwan) citizen with household registration, and the applicant's parents must currently have household registration in the R.O.C. (Taiwan), one original Household Registration Certificate issued within 3 months that shows registration of the parents' marriage is required.
- For applicants who are under age 18 and are the children of an R.O.C. (Taiwan) citizen without household registration, a foreigner having obtained APRC or ARC or other kinds of residence permits, a Hong Kong or Macau resident, or a Mainland Chinese citizen, proof of parent's marriage is required.

| Other supporting documents |
- Applicants with dual citizenship and household registration in the R.O.C. (Taiwan) must annul the household registration and enter the R.O.C. (Taiwan) with a foreign passport.
- Applicants whose parents have divorced may only join the parent that has custody of the applicants. Proof of custody is required.
- Applicants born outside of the R.O.C. (Taiwan) and whose parents are R.O.C. (Taiwan) citizens having household registration must present the original and one photocopy of both parents' R.O.C. (Taiwan) passports.
- Applicants who apply to join paternal or maternal grandparents who are R.O.C. (Taiwan) citizens having household registration must present a letter of authorization signed by the applicant's father, mother or legal guardian.
- Applicants who apply for a Resident Visa after having arrived in the R.O.C. (Taiwan) are required to present also one photocopy of the Visitor Visa and the immigration entry stamp.
